A systematic review of preclinical evidence by the ESSKA Innitiative on corticosteroid injections for the treatment of OA
The systematic review of preclinical evidence conducted by the ESSKA Orthobiologic Initiative evaluated the effects of intra-articular corticosteroid (CS) injections in joints affected by osteoarthritis (OA). Analyzing 32 studies in animal models (1079 joints), it was found that CS injections showed positive effects in 68% of the studies and detrimental effects in 16%. Cartilage and synovial improvements were observed in 68% and 60% of studies, respectively, but negative effects were also documented in 11% and 20%. In terms of pain, lameness or inflammation, there were improvements in 63% of studies and worsening in 13%. Evidence on imaging, biomarkers, CS type, dose, formulation, and injection protocol is limited. Risk of bias was low in 28% of studies and high in 18%.
The review concludes that CS injections can have a wide range of effects on OA joints, from short-term benefits to negative effects. It highlights the need to identify specific indications and treatments to maximize benefits and avoid detrimental effects.
